Disclosure Statement
The following fields are required.
Having an interest in or affiliation with any corporate organization does not prevent a speaker from making a presentation, but the relationship must be made known to the audience in advance. The Accreditation Council for Continuing Medical Education (ACCME) requires that SCA ensure that the following decisions were made free of the control of a commercial interest. (The ACCME defines a "commercial interest" as any proprietary entity producing health care goods or services, with the exemption of non-profit or government organizations and non-health care related companies.)
I agree to comply with ACCME Standards for Commercial Support regarding independence in planning CME activities as listed above, choosing the best available speakers and, when possible, avoiding speakers who might have conflicts of interest. Yes No Not Applicable (to presenters)
I agree that my presentation will be unbiased, based on the best available evidence and that all elements of the presentation will be free from the control of commercial interests Yes No Not Applicable (to planners)
SCA must be able to show that everyone who is in a position to control the content of an educational activity has fully disclosed all, his/her and/or his/her's immediate family, relevant financial relationships with any commercial interest to the provider. The ACCME defines "relevant financial relationships" as financial relationships in any amount occurring within the past 12 months that create a conflict of interest.
An individual must fully disclose to the learners any relevant financial relationship(s). If there are no relevant financial relationships, the learners must be informed that no relevant relationship exists. An individual who refuses to disclose relevant financial relationships will be disqualified from being a planning committee member, a teacher, or an author of CME, and cannot have control of, or responsibility for, the development, management, presentation or evaluation of the CME activity.
SCA will implement a mechanism to resolve all conflicts of interest before the educational activity takes place.
Does the Planner/Presenter or his/her immediate family members have a financial relationship as described above? Yes No I refuse to disclose this information
If you answered yes, please complete the fields below by entering the company name and checking the appropriate boxes. (Note: if you disclose a relationship with industry, a member of the Planning Committee will contact you to develop a plan for the resolution of the conflict.)
Generic and Trade Names Presentations must give a balanced view of therapeutic options. Presenters' use of general names will contribute to this impartiality. If trade names are used, those of several companies should be used rather than only that of a single supporting company
Unlabeled Uses of Products When an unlabeled use of a commercial product, or an investigational use not yet approved for any purpose is discussed during an educational activity, the presenter is required to disclose that the product is not labeled for the use under discussion or that the product is still investigational.
Discussion of unlabeled uses: Yes No If yes, you must disclose this information during your presentation.
I have read the CME requirements and agree to adhere to these requirements. I agree
